1. ASSISTANCE TO INDIVIDUALS IN CRISIS SITUATAIONS
Assistance to Individual in Crisis Situation or AICS is a form of assistance funded by the local government unit of Burgos that addresses the immediate and emergency need of an individual or family. Emergency in nature includes purchase of medicine, augment hospital bill, and transportation and food assistance.

2. ISSUANCE OF SOCIAL CASE STUDY REPORT TO DIFFERENT AGENCY CONCERN/PCSO/HOSPITALS
Social Case Study Report (SCSR) is a document that describes the present situation of a needy individual or family for possible availment of medical, burial, educational, and financial assistance or intervention from charitable institutions (GOs & NGOs). It is done by a registered social worker through a conduct of interview and data gathering. It justifies the current condition of a client to be eligible for an assistance from sponsoring agencies.

3. APPLICATION FOR IDENTIFICATION CARD FOR SENIOR CITIZEN, PERSON WITH DISABILITY (PWD) AND SOLO PARENTS
· FOR SENIOR CITIZEN
Senior Citizen’s Identification Card is issued to residents of this Municipality whose age is 60 years old and above. Once a person reaches his 60th birthday, he should apply for a Senior Citizens Identification card (ID) in order to avail 20% discount of medicine, medical and dental check up, transportation, hotels, restaurants and other privileges granted from R.A. 9994.
· FOR PERSON WITH DISABILITY
A document issued to Persons with Disability under Section 32 of RA No. 9442, “An Act Amending RA No. 7277, otherwise known as the ‘Magna Carta for Disabled Persons and for their Purposes’.
· FOR SOLO PARENT
A document issued to Solo-Parent according to Republic Act 8972.

FOR SENIOR CITIZEN Any Filipino citizen who is a resident of the Philippines, and who is sixty (60) years old or above per Article 5.1 Rule III, definition of terms. FOR PERSON WITH DISABILITY Any person suffering from long-term physical, mental, intellectual or sensory impairments and a resident of the municipality. FOR SOLO PARENT Any person qualified as to the definition of Solo-Parent in accordance to RA 8972.
